My Record of Achievement
4-H members in grades 3-12 are asked to complete a "My Record of 4-H Achievement" form each year by October 1. This is a way for you to reflect on your accomplishments and organize your records for 4-H scholarship/award applications.
In order to be invited to serve on the Ambassador Board, members must submit a "My Record of Achievement" Form. Those members with the greatest amount of 4-H activities, projects, etc. are invited to join the Ambassador Board.
