The following are forms that are general for many projects. Please feel free to print out the forms you may need.
We are currently looking for ways to improve the Jay County 4-H program and ways to benefit our members. Jay County 4-H requires to have one Record Sheet per member for all building projects. Each project, manuals, and record sheet needs to be looked over your the Club Leader. Each record sheet must be signed by the club leaders, stating the project and manuals were complete. RECORD SHEETS are due at CHECK IN of projects.

We have added the record sheets for you to print out per age group. You can also print out the building project labels as well.
