For 4-H Project Superintendents: 

4-H Project Committee Form - due by February 21st - download and complete the form below and return to our office via mail or email (, or complete the web-based online form here

Workshop Sign-In Form - Please use this form to record workshop attendance.  Only 4-H members need to sign in.  The information gathered is used in our annual federal reporting to maintain the support of the 4-H Program.  Please return these forms to our office via mail or email following each workshop.

Superintendent Commitment Form - Please read the document below entitled "2019-20 Superintendent Re-Commitment Info" before completing the form at If we do not receive your response by February 21st, we will assume you wish to resign your position and we will proceed to appoint a new volunteer to serve as superintendent.
