Fall 2019 4-H Achievement Awards

MARK YOUR CALENDAR: This year's 4-H Achievement Night will be held on Tuesday, October 15th at 5:30 p.m. at Expo East at the Fairgrounds (enter on the north end of the building). More details about the dinner menu will be posted soon. Please come and enjoy dinner, fellowship, and fun with 4-H families and friends from around the county. We look forward to seeing you! Following are descriptions of our various Porter County 4-H Achievement Awards. Nomination and application forms are available for download at the bottom of this page. Deadline for all applications is September 20th, 2019.

 

4-H RISING STAR AWARD: This award is for 4-H members who are currently enrolled in grades 4, 5, or 6. Members must be nominated by their 4-H Club Leader on a form which is available for download below or using our all-new web-based form. If you're interested in the award, don't be afraid to let your Club Leader know. There is no limit to the number of 4-Hers who can receive the "4-H Rising Star" award each year. Members may be nominated to receive the award each year they are eligible. Recipients of the award will receive a 4-H gift at the fall Achievement Day.  (Nomination form available for download at the bottom of this page OR use our all-new web-based form at this link.)

Submission Checklist for Rising Star Nominations (due by 9/20/19):

  1. OPTION 1: Complete web-based form by 9/20/19 at 11:59 p.m.. -OR-
  2. OPTION 2: Download fillable form (below)
    1. Save/complete form on your computer and email completed form to joangrott@purdue.edu by 9/20/19 at 11:59 p.m. -OR-
    2. Print/complete and turn in paper to the Extension Office by 9/20/19 at 4:30 p.m.

 

4-H JUNIOR AMBASSADOR AWARD: This award is for 4-H members who are currently enrolled in grades 7 or 8 and have completed at least two years of 4-H. Members must apply for this award on an application form that is available for download below or using our all-new web-based form. Members may reapply for the award each year they are eligible. First-time recipients of the award will receive a chenille 4-H patch (letter jacket style) at Achievement Day. Second-time recipients will receive a 4-H gift as well. Individuals who are named Junior Ambassadors are encouraged to volunteer to assist with various 4-H activities and events at the following year's Porter County Fair. Submission Checklist for Junior Achievement Ambassadors (due by 9/20/19):

  1. OPTION 1: Complete web-based form by 9/20/19 at 11:59 p.m.. -OR-

    1. Save/complete form on your computer and email completed form to joangrott@purdue.edu by 9/20/19 at 11:59 p.m. -OR-
    2. Print/complete and turn in paper to the Extension Office by 9/20/19 at 4:30 p.m.

 

4-H ACHIEVEMENT AMBASSADOR AWARD: This award is for 4-H members who are currently enrolled in grades 9-12, completed at least four years of 4-H, and have at least one year of 4-H eligibility remaining (i.e., you must be enrolled in 4-H during the 2020 fair season). No more than two "Achievement Ambassadors", no more than two "Reserve Ambassadors", and no more than six additional members of the "Achievement Board" will be selected from those who complete the application process. The application process includes submission of an application form and a letter of reference, as well as participation in an interview with the Ambassador judges on {DATE COMING SOON}. Individuals who are named to the Achievement Board will be expected to assist with various 4-H activities and events in the year ahead, particularly at the Porter County Fair.
