All adults working with children through the Vanderburgh 4-H Program are required to go through an application, background check, and approval process.
To begin the process, download and complete the application below. Please fill it out completely, including an address or email address for each reference. Once your application is received by the Extension Office, we will send out letters to be completed by each of your references and make sure you have viewed the Child Protection PowerPoint and Handout (found below). After we receive your references forms back, the Extension Educator will check the Indiana Sex and Violent Offender registry. Once the process has been completed, you will receive notification and a certificate.
