Achievement Records

STATE FAIR ENTRY: No

JUDGING TIME: Due to the Extension Office by August 15.

GENERAL GUIDELINES
--The 4-H member (not the parent) should do the report. You may ask parents for help.
--Handwrite, print or type neatly. Any handwriting needs to be done by the 4-H member, regardless
of perceived neatness.
--Show totals in LEAD PENCIL because as you add to your record year by year, the totals will
change.
--Put each item or activity you’ve done in only one place; don’t repeat.
--Do NOT add pictures, ribbons, etc. to this report. Keep them in a 4-H Memories box.
--Extra copies of pages 6 and 7 can be obtained at the Extension Office.
--Copies of other pages may be made if needed. Remember, the books are not judged better because
they are thicker. We discourage adding pages unless absolutely necessary.
--When done, place in a folder of your choice. Put your name, township and grade on front.
--Turn in to the Extension Office by August 15.
