Any youth enrolling in Warrick County 4-H should be in grades 3 thru 12 as of January 1st of the current school year.  Youth in grades K-2 will enroll in the Mini 4-H program. They must complete an enrollment form either online or a hard copy and pay the enrollment fees (see enrollment fees below). Enrollment is not considered complete until all membership fees have been paid. For hardship circumstances, please contact the office. Youth in grades K-2    

NOTE: Manuals must be picked up at the Extension Office. One manual will be given for each project per level or division. Please re-use your manuals as long as the 4-H'er is in the same level. There will be a $5.00 charge for duplicate manuals.

Online Enrollment Instructions - A guide to help you navigate the online enrollment for Returning Members and New Members.

Enrollment Form - If you choose to download the hard copy of the enrollment form, you will need to bring it into the office along with your payment.

Youth Health Form- If your 4-H'er will be participating in activities at the Warrick County 4-H Fair, we will need to have a current Youth Health Form on file in the Extension Office. (We need a new one each year. For other events you will need to fill out another health form.) 

4-H Enrollment Fees - These fees are to enroll in Warrick County 4-H; clubs may have an additional fee. If you need more information on the enrollment fees please contact the Extension Office. 

Mini 4-H Enrollment Fees - Fee for Warrick County Mini 4-H Club is $5.00 per Mini 4-Her.  This is not payable online and will have to be paid at the Extension Office or the first Mini 4- H Meeting attend by the Mini 4-Her.
