CALEY MEMORIAL 4-H AWARD
Application due at County Extension Office by: June 23, 2023
This award will be given annually to two 4-H members in Wells County who have shown outstanding
qualities in the area of leadership, citizenship, sportsmanship, and personality in their community, school,
church, and 4-H activities.
A. NOMINATION PROCESS
1. All current 10 year or final year (due to age) members can find an application form on our
website www.extension.purdue/wells or the Wells County 4-H Facebook page which
they can submit.
2. In addition, the 4-H Council Awards Committee and the Extension Educator can suggest
outstanding 8th or 9th year member. Members can find an application form on our
website www.extension.purdue/wells or the Wells County 4-H Facebook page. Members
must be at least 14 years of age.
3. Interested parents, leaders, adults, and 4-H members can pick up an application at the
Extension Office.
B. THE APPLICATION
1. The application will cover 4-H, school, community service, church, or other youth groups.
2. The first page of the application will have general information about the applicant and will not
be removed during the selection process. Applicant’s name should not appear on the
remainder of the application.
3. The Applicant will be given three recommendation forms, two to be completed by an adult
and one form to be completed by your 4-H Club Leader. To speed the process along, you may
want to send a self-addressed, stamped envelope for these individuals. Have the form mailed
directly to the Wells County Extension Office,1240 4-H Park Road, Bluffton, IN
46714.
4. The application form and recommendation forms are for the use of the selection committee.
Names will remain confidential except for the winners.
5. Do not add any additional pages to the application.
C. THE SELECTION
1. The overall 4-H record will be considered. Number of projects and completion should not be
the only factor considered. Other things considered will be offices held, committees,
volunteer service and leadership activities.
2. Judging of the applicants will be based on the following:
45% - 4-H 15% Community Service
15% - School 15% - church or Youth Group
10% - Other
3. Final selection will be made by an anonymous committee of five individuals appointed by the
President of the 4-H Association and County Extension Educator.
4. The award can be awarded only once to any one individual.
D. THE AWARD
1. The award will consist of a $500 cash scholarship and a plaque given to each winner. In
addition, the 4-H member’s name will appear on a plaque kept at the 4-H Park. Two awards will be selected each year.
